This form is only for students who have received their official admission decision in the mail or online and wish to discuss that decision with a counselor. Inquiries will become part of the applicant's permanent record.

Transfer students please note: Under the regulations of the Family Educational Rights and Privacy Act (FERPA), we will only discuss decision iquiries with the applicant.
